The Family Success Team Model

The MCP Family Success Team (FST) process starts with the family or individual completing a comprehensive strength-based assessment to identify areas of need, strength, and interest. MCP staff, a "Community Host", then works with the Monument resident/family. Timely, measurable and desired goals and outcomes are developed by the family with the assistance of the Host. Based on the needs and goals, a unique team of representatives from a variety of agencies are identified to guide and assist the resident/family. MCP brings together "specialist" agencies to meet with the family and create an achievable plan, the Family Partnership Agreement. The MCP Host acts as a generalist, assisting with navigation through the service spectrum and providing continual follow up and motivation. The Host often acts as a coach/mentor supporting a resident through their change process. Each "specialist" works on a specific issue with the family. Any agency representative on the team can take the lead, with MCP providing the detailed follow up and coordination. The team for each family meets on a regular basis, every three months to review the progress of the family and revise the service plan as needed. The response from participating families has been overwhelmingly positive as they build confidence and have real success in achieving their goals.

Eligibility: Families or individuals must reside in the Monument Corridor Community.
